Intake Clerk

Revival Home Health Care is actively looking to hire Intake Clerk to work for our Certified Home Health Agency.

Responsibilities:

• Answers telephone calls directed to the intake department and prioritizes calls
• Obtains demographic information from referral sources and assigns the referral to an intake coordinator
• Enters intake data into the computerized system with 100% accuracy
• Serves as liaison for the Intake Coordinators working in the hospitals or nursing homes
• Notifies all appropriate departments of any changes related to the SOC
• Forwards insurance information to the Insurance Verifier, and ensures timely verification
• Organizes SOC information and assures placement of COC and ancillary services
• Performs all clerical functions of the intake department
• Maintains NTUC log, ensuring all appropriate departments are notified of the NTUC
• Collaborates with Director of Intake, Supervisors, and Coordinators. Consults with the supervisor when appropriate
• Represents Revival effectively and treats all customers courteously and in a non-discriminatory manner
• Performs other related duties as required
